Requirements Needed For Applying PDEA S-License For Drug Establishment (New Applicant)

Application Sheet should be attached on the first page. Green folder should be used in submission of requirements. The following are requirements:
  1. TIN Card/ Latest ITR (Head of Office & Authorized Pharmacist)
  2. NBI Clearance (Head of Office & Authorized Pharmacist)
  3. PRC ID Card (Authorized Pharmacist)
  4. PTR (Authorized Pharmacist)
  5. TIN Card/ Latest ITR (Company)
  6. DTI/ SEC/ CDA Certificate
  7. General Information Sheet – GIS (corporations only)
  8. Business/Mayor’s Permit
  9. FDA or DOH - CHD LTO Certificate
  10. BOC Accreditation Certificate(importer/exporter only)
  11. *Projection for forthcoming year (importer/ manufacturer only)
  12. Certificate of Product Registration – CPR (importer/ manufacturer/ exporter only)
  13. Picture of DDPs/ Box Label/ Package Insert (importer/ manufacturer/ exporter only)
  14. Company profile
  15. Organizational structure, certified correct and current by Human Resource (HR) Officer of company with pictures of head and authorized pharmacist
  16. Location/Vicinity Map
  17. Floor plan/ Layout to highlight controlled drug storage area
  18. Picture of Drug Establishment (façade)
  19. Picture of controlled drug storage area
  20. Proof of ownership/ Lease of Contract 

    *Original current document required. For the rest of the requirements, submit clear photocopy and present original for validation.
